• Shift: Day Shift
• Hours: Clinic hours, 8am-4pm, Monday through Friday (Provider will work 4 days/week)

License/Certification:

• Nurse Practitioner:

  • Minimum: Registered Nurse Licensure from the state of Indiana.
  • Nurse Practitioner certification.
  • Basic Life Support (BLS) certification required.

• Physician Assistant:

  • Minimum: Physician Assistant Licensure from the State of Indiana.
  • National Commission on Certification of Physician Assistants (NCCPA) certification.
